Requirements to serve:

  1. Download, complete, and mail Application to LOT318. Click here for Application.
  2. Download, complete, and mail Covenant to LOT318.  Click here for Covenant.
  3. Download, complete, and mail Release to LOT318.  Click here for Release.

Please mail all three forms to:

Once all forms have been submitted Lot318 will contact you regarding a time to serve.

My daughter is looking for a volunteer position. She is 17 years old and attends Canyon High School. Please let me know some information on age requirements to help in afternoons with the kids I would appreciate it.

Awesome! Go ahead and download the application, covenant and release form and send that in to me. All the contact info. is on lot318.com, as well as our volunteer opportunities and calendar. We have HW Club Mon-Thurs. from 3:30-5:30 as well as other monthly opportunities. Look forward to serving with you!
